The match at the Toyota Stadium in Bloemfontein starts at 16h30 and will be live on the SuperSport Rugby and Grandstand channels.
Rhyno Smith comes in at right wing, while former Emirates Lions and Southern Kings midfielder Howard Mnisi is entrusted with the No 13 jersey.
Smith gets the nod over Malcolm Jaer, while Mnisi rotates with William Small-Smith.
There are also two changes upfront for the Free State outfit, with Hencus van Wyk (tighthead prop) set to make his full debut in place of former Junior Springbok prop Luan de Bruin, while Boan Venter and Charles Marais swap places at loosehead.
Meanwhile, Toyota Cheetahs captain Junior Pokomela has been bracketed with former Southern Kings skipper Aidon Davids, with Davids confirmed on the bench should Pokomela be declared fit to lead the side.
Last weekend, the Bloemfontein side suffered a 30-13 loss to the DHL Stormers in Cape Town, their second successive defeat in the competition following impressive victories in the first two rounds.
The Free Staters are currently fourth on the log with 12 points and would therefore want to end the competition on a positive note with a win over their neighbours.
The following players are not eligible for team selection due to injury: Wilmar Arnoldi (cheek bone), Reniel Hugo (knee), Aranos Coetzee (ankle), Ruan Pienaar (knee), Craig Barry (knee), Luan de Bruin (bicep) and Charles Marais (neck).
